3 Games of pool play – seeding into bracket play - single elimination

May 5, 2017  PROOF of ASA registration (receipt registration or Bollinger Insurance Certificate)
May 19, 2017 – Hotel Confirmation Form (2017 14u)  (form - hotel chosen and number of rooms reserved) out of area only.
June 5, 2017 – #1 Team Roster Form and #2 Summary of Wrist Band by Graduation Year (f/ College Coaches Booklet)

(1) Fill in the Application Form and click "Submit Application"
(2) MUST submit all PAPERWORK with check to be considered registered

YOU ARE NOT REGISTERED UNTIL CHECK IS CASHED WITH PAPERWORK

 
  1. Application that you are or will be a registered 2017 14u ASA team with check and once approved and accepted for the 2017 (14u) Champions Cup … we will cash your check. (Uncashed checks means you are on a waiting list)

    One or the Other:
    Provide proof of 2017 ASA registration (receipt) or 2017 certificate of Bollinger Insurance
  2. Signed Competition Agreement that you agree to all terms and conditions to play in the Champions Cup (mail with application)
  3. Signed Hotel Agreement that you agree to all terms and conditions to play in the Champions Cup (out of Area teams mail with application)
  4. A Hotel Confirmation Form will be emailed to the teams' contact (with hotel prices) and must be retured (with the hotel chosen and number of reserved rooms prior to May 19, 2017 - or sooner when you have made your reservations) or submit an additional fee of $700 with the application fee if your team DOES NOT STAY IN one of our 2017 16u approved hotel list - (view Irvine Hotel List)
  5. Roster Form – fill in CHAMPIONS CUP FORMAT (prior to June 5, 2017) if you want to be included in the College Coaches Booklet …. Please type with Initial Caps)

    Page 2 of Roster Form -- (20 maximum) WRIST BAND LIST (year of high school graduation ie., 2018, 2019, etc., signed/verbal, or unsigned)

    • Color wrist bands will be included in the coaches packets with your form as packing slip
    • Any additional lost/forgotten wrist bands could cost $2 each at the fields

The 2017 14u, 16u & 18u/Gold Tournament are open for applications. Please click on the Online Registration Form to start the process. DO NOT REGISTER UNTIL YOU ARE READY TO PRINT, SIGN AND RETURN TO THE ADDRESS BELOW WITH A CHECK.

IMPORTANT: All applications must include: printed Online Registration Form, Competition Agreement and Hotel Agreement if you are from out of the area, Manager Meeting if you want to attend the dinner and A CHECK to be considered. See instructions below:

Step 1: Fill in the Online Registration Form (make sure data is correct for contact purposes)
Step 2: Click "Submit Application"
Step 3: Read and sign Competition Agreement
Step 4: Read and sign Hotel Agreement if you are from out of the Area (schedule hotel stay one day prior to Game Day as games begin at 8:00AM)
Step 5: Read and sign the Manager Meeting Form if you are attending the Dinner, and add appropriate amount to the Tournament fee
Step 6: Sign a check payable to: The Champions Cup
Step 7: Payment with Paypal requires an additional $30 fee and direct contact with Susan Bollinger for an invoice and requires scanned or mailed paperwork.
Step 8: Mail all required paperwork including a check to:
